Below is the procedure for processing the Additional Information Template and Forms for 'SSN Needed' profiles.  This comes from Real-time Registration Background Checks that come back with a result needing a Social Security Number to be processed.


Go into the Background Check list for you profiles that need a SSN, select them and choose the 'Email Selected' action.  That brings you to the email editor where you can compose the email to be sent.  A template  has been created that you can drop into the body of the email message that contains this:


Dear School District Parent or Community Member,


Thank you for completing a volunteer application to serve in our Schools.  Your willingness to volunteer is appreciated.  In order to process your background check, we need to collect more information from you.  Due to the commonality of your name, the Volunteer system will additionally need your social security number to continue processing the background check to eliminate similar names and birth dates. 


This information is collected and processed through a secured web address. Since this allows the scope to be better narrowed, processing time should run quickly.

When you prepare the message and hit Send at the top, it will tell you that the messages were added to the queue and then you can hit Finish.


At this point, it is recommended to move these profiles into the 'SSN Requested' Backgroudn Check List to show that an email has been sent to them.


When a volunteer completes the Form, the Volunteer system will email a message that looks like this:


Question # 1: First Name:
Answer: Sally

Question # 2: Middle Name:
Answer: Test

Question # 3: Last Name:
Answer: Example

Question # 4: Date of Birth (mm/dd/yyyy):
Answer: 01/01/1980

Question # 5: Aliases or previous names. Include First and Last Name only. One previous name per line.
Answer: 

Question # 6: Social Security Number:
Answer: 123-45-6789


**Only one email can be added to receive the form responses, and that is set in the Form itself.


After receiving one of these messages you go back into the Background Module and find the list that has the profile.  You can use the Search box at the top of the list page to help find on which list the profile is.  From there you select the profile by checking the box on the left and choosing the action 'Resubmit with SSN'.  You can only apply this action for one profile at a time.


Again, the system will submit the review and then update the profile with a result.  From there you can treat it as the others; moving it to the Further Review list or removing it if it's OK-Green.
